APPLICATION PROCEDURE


Timely Submission of Proposals


Proposals are accepted on a rolling basis and considered by the Board quarterly. The following deadlines apply to each quarterly review:

DEADLINES ARE FIRM. Applications that are not received by the above deadlines will be moved to the next grant cycle. Applications that are incomplete will be declined unless completed within 10 business days of initial receipt. If you do not receive an acknowledgement of your proposal, please contact our offices to verify that your submission has been received.


A limited number of proposals of $20,000 or less are accepted and considered on a rolling basis. The above deadlines do not apply. For more information, please contact our office,

II Method of Submission


All proposals MUST be submitted electronically to This e-mail address is being protected from spambots. You need JavaScript enabled to view it , with exceptions for the supporting documents that are asterisked below.  When emailing large files, you may need to send more than one email.


III Content of Proposals

Proposals must include the following:

1. COVER SHEET & PROPOSAL SUMMARY of no more than one paragraph, including a completed NY/NJ Application Form


2.  NARRATIVE - five pages maximum, including, in the order indicated:


a) Brief (one paragraph) description of agency mission and history, population served, geographic and socio-economic focus, and relevant current initiatives

b) Identify the overall goal of the proposed project, the need it is designed to address, and its target population

c) Indicate the specific objectives of your proposed work, the specific activities to be undertaken to reach each objective, and the staffing required for the project. Include a timeline for the proposed activities.

d) Discuss the impact you plan to achieve, including both process outcomes (i.e., number of sessions held, people served, etc.) and change outcomes (improvements in health, changes in behaviors, etc.)

e) Budget narrative: explain the major lines of the HFNJ Grant Application Budget, which should be attached to your proposal as indicated in #4 below.

f) Set forth your evaluation plan, referring to the HFNJ Evaluation Logic Model (see #3 below). What methods of data collection will you use to measure the outcomes achieved? What instruments, observations, recording-keeping, etc.?  Information to assist you in developing the Logic Model.

g) Set forth a plan for sustainability. How will this project continue once funding from the HFNJ is no longer provided?

5. RESUMES OF KEY STAFF, not longer than one paragraph each, including relevant qualifications.

6. AGENCY-SPECIFIC SUPPORTING MATERIALS:

a) *Most recent audited financial statements, reflecting actual expenditures and funds received during your most recent fiscal year, including management letter.

b) *Unaudited financial statements if your audited statement is not completed for the most recent fiscal year.

c) *Most recent form 990, if not available online through Guidestar or The Foundation Center

d) *Copy of your most recent IRS determination letter indicating your agency’s tax exempt status

e) Your agency’s operating budget for the current fiscal year, including revenues and expenditures

f) List of grant support from other outside sources over the past three years

g) List of your Board of Directors, with their affiliations

h) *Your most recent annual report/brochure, if applicable

i) *No more than three examples of recent articles about, or evaluations of, your organization (optional)

j)*Letter(s) of support from any agency(ies) to be partnering with you on the proposed project



*May be submitted by mail, but must reach the Foundation by the application deadline.
